You can also try a variation of this test where the patient resists further internal rotation, adding an isometric external rotation force. Self- resisted isometric hip horizontal abduction in sitting. This test aims to compress the sciatic nerve in the deep gluteal space through contraction of the piriformis and other deep rotators. You can also try a variation of this test where the patient simultaneously squeezes their heels together, adding some additional stimulus to the short external rotators.
